目的 探讨胸腔镜手术在双侧肺部结节性病变中的应用价值.方法 2010年9月至2015年12月期间收治16例患者,经胸部CT检查证实双肺均有结节性病灶,于胸腔镜下同期手术,具体术式为肺楔形切除术、肺段切除术或肺叶切除术.结果 16例患者无死亡病例,手术时间平均195min,术中出血平均150ml;胸腔引流管留置时间平均5.2d.术后住院时间平均13.6 d.其中1例行二次手术,后经治疗顺利出院,其余患者术后恢复良好.结论 胸腔镜下双侧肺同期手术相比于分期手术,缩短了治疗时间,节约了医疗成本,使疾病治疗更加及时,对于双肺结节性病变且全身情况可以耐受者,胸腔镜下双侧肺同期手术是一种较好选择.%Objective To investigate the value of video assisted thoracic surgery in bilateral pulmonary nodules. Methods Retrospective analysis of 16 patients who were treated from September 2010 to December 2015.The double lung nodules were confirmed by Chest CT examination, and were removed during the same period from the VATS.Surgical methods included pulmonary wedge resection or segmentectomy or lobectomy. Results There was no mortality in 16 patients,the average operation time was 195min,average bleeding was 150ml;the average time of pleural drainage tube was 5.2d.The average hospitalization time was 13.6d.One of the patients underwent surgery again,and discharged after treatment,the other patients recovered well. Conclusions Compared with staged operation from the VATS,bilateral lung surgery shortens the treatment time,saves the medical cost and makes the treatment more timely. Therefore,a patient with nodules in bilateral lungs and his body can tolerate surgery,bilateral pulmonary surgery at the same period from the VATS is a good choice.
