Objective To explore the clinical curative effect of supplementing Qi and nourishing Yin method combined with conventional western medicine treatment in the patients with diastolic heart failure complicated with atrial fibrillation. Methods 85 patients with diastolic heart failure and atrial fibrillation were randomly divided into two groups,45 cases in the treatment group,40 cases in the control group,two groups of patients were treated with conventional Western medicine,and the treatment group was treated with supplementing Qi and nourishing Yin method, the course of treatment was 4 weeks. Before and after treatment with carvedilol,NYHA class,6 minutes walk test,measurement and analysis of the changes of serum homocysteine(Hcy),plasma B-type brain natriuretic peptide(BNP)and observation of cardiac diastolic function indexes by color Doppler ultrasound. Results In the treatment group,heart function(NYHA)and 6 minutes walk test,BNP and Hcy were significantly improved (P<0.0001) compared to the control group,the difference was statistically significant(P<0.01).In the treatment group the cardiac ultrasound showed mitral E peak(left ventricular diastolic early peak velocity),A peak(left ventricular diastolic late peak velocity),E / A ratio,left atrial diameter(LAD)and shoot ejection fraction are improved compared with the control group,it was statistically significant. Conclusion Supplementing Qi and nourishing Yin combined with conventional western therapy can effectively improve the heart function indexes and reduce the atrial fibrillation recurrence or reduce the risk of atrial fibrillation occurred.%目的 探讨益气养阴法联合常规西医治疗舒张性心力衰竭(DHF)合并心房颤动的临床疗效.方法 将85例DHF合并心房颤动的患者随机分为两组,观察组45例,对照组40例,两组患者均予以常规西医治疗,同时观察组加以益气养阴法治疗,疗程均设为4周.治疗前后对两组患者进行NYHA心功能分级,进行6min步行试验(6MWT),测定分析血清同型半胱氨酸(HCY)和血浆B型脑利钠肽(BNP)的变化情况,采用彩色多普勒超声观测心脏舒张功能指标的改变情况.结果 观察组心功能NYHA分级及6MWT距离,BNP及HCY均得到明显改善(P<0.0001),与对照组比较,差异均具有统计学意义(P<0.01),心脏超声显示观察组二尖瓣E峰值(左室舒张早期流速峰值)、A峰值(左室舒张晚期流速峰值)、E/A比值、左房内径(LAD)和射血分数均有改善,与对照组比较差异有统计学意义(P<0.05).结论 益气养阴法联合西医常规治疗可有效改善心功能相关指标,降低房颤再发及房颤并发症发生的风险.
