目的 观察原发性肝癌(肝癌)与慢性乙型肝炎(乙肝)患者中,血清同型半胱氨酸(Hcy)的变化规律,及其对疾病的诊断价值.方法 收集肝癌、乙肝患者为观察组,健康体检者为对照组.对各组人群进行血清Hcy检测,对检测结果进行比较分析.结果 肝癌组血清Hcy(19.30±6.47)μmol/L,乙肝组血清Hcy(11.93±4.73)μmol/L,对照组血清Hcy(9.87±3.68)μmol/L,三组比较差异均有统计学意义.肝癌患者中,男性Hcy值显著高于女性(P<0.05);乙肝组与对照组男女间Hcy值比较,差异无统计学意义.结论 肝病患者血清Hcy升高明显,将Hcy用于肝癌的实验室检查有一定的临床价值.%Objective To observe the variation and diagnostic value of serum homocysteine(Hcy) in primary liver cancer(hepatocellular carcinoma)and chronic hepatitis B(HBV) patients.Methods Liver cancer and hepatitis B patients were collected as the observation group, healthy people as a control group.The levels of serum Hcy of each group were detected,the test results and receiver operating characteristic curve (ROC) were compared and analyzed.ResultsHCC serum Hcy was 19.30±6.47μmol/L,serum Hcy hepatitis B was 11.93±4.73μmol/L, serum Hcy in the control group was 9.87±3.68μmol/L,had statistically signifi cant differences between the three(P<0.05).95% confi dence interval upper limit of detection of the control group Hcy values(10.83μmol/L)as the critical value,high Hcy hyperlipidemia incidence of liver disease group was significantly higher,the difference between the three was statistically significant(P<0.05).In HCC patients,Hcy values were higher in male than that in female,the difference between t he two was statistically signifi cant(t=2.593,P<0.05). In hepatitis B group and control group,Hcy values between men and women,the difference was not statistically signifi cant(P>0.05).AUC HCC group was 0.878,SE to 0.025,P was 0.000,95%CI 0.828-0.928 for the best critical value 12.41μmol/L,the sensitivity was 88.20%,a specifi city of 78.00%.The AUC hepatitis B group was 0.370,no diagnostic signifi cance.ConclusionSerum Hcy increases signifi cantly in patients with liver disease,the Hcy for the diagnosis of liver cancer has some clinical value.
