探讨RT-3DE评价急性心肌梗死患者左室功能及收缩同步性的可行性

         

摘要

目的利用实时三维超声心动图容积-时间曲线评价急性心肌梗死患者左室功能及收缩同步性.方法采集63例急性心肌梗死患者(心梗组)及38例健康者(正常组)的三维图像,用Qlab(8.1)软件分析出各参数,将两组各参数进行比较.结果心梗组左室舒张末容积(EDV)、左室收缩末容积(ESV)、大部分节段舒张末容积(rEDV)、节段收缩末容积(rESV)、16节段达最小收缩末容积时间的最大差值、标准差及所在心动周期的百分比(Tmsv-16-Dif、Tmsv-16-Dif%、Tmsv-16-SD、Tmsv-16-SD%)大于正常组(P<0.05),心梗组射血分数(EF)、节段射血分数(rEF)小于正常组(P<0.05),心梗组EF与Tmsvl6-Dif、Tmsvl6-Dif(%)、Tmsvl6-SD、Tmsvl6-SD(%)呈负相关(P<0.05),心梗组异常节段的rPER、rPFR小于正常组(P<0.05).结论实时三维超声心动图可以定量评估急性心肌梗死患者左室功能及左室收缩同步性.%Objective To quantitatively assess global and regional left ventricular function and systolic synchronicity in patients with acute myocardial infarction(AMI)using volume-time curves on real-time three-dimensional echocardiography(RT-3DE). Methods RT-3DE images were acquired in Sixtiy-three patients with AMI(myocardial infarction group)and thirty-eight normal subjects(normal group),Each parameter were measured by the Qlab(8.1)software ,compared each parameter between the two groups. Results The left ventricular end diastolic volume(EDV)、left ventricular end systoli volume(ESV)、most of the regional end systolic volume(rEDV)、regional end diastolic volume(rESV)、the standard deviation and maximal difference of the time to minimal systolic volume in 16 segments and the percentage in a cardiac cycle(Tmsv-16-Dif、Tmsv-16-Dif%、Tmsv-16-SD、Tmsv-16-SD%)in myocardial infarction group were more than those in normal group(P<0.05).The left ejection fraction (EF)、regional ejection fraction(rEF)in myocardial infarction group were less than normal group(P<0.05).In myocardial infarction group ,the EF and Tmsv-16-Dif、Tmsv-16-Dif%、Tmsv-16-SD、Tmsv-16-SD%have negative correlation(P<0.05). The PER、PFR of abnormalitic wall motion in myocardial infarction group are more than normal group(P<0.05).Conclusions RT-3DE can quantitative assessment left ventricular function and systolic synchronicity in AMI patients.
