3.0TMR功能成像评价与预测乳腺癌新辅助化疗疗效的分析

         

摘要

Objective To Explore the indicators change characteristics in breast cancer before and after neoadjuvant chemotherapy(NAC) in 3.0MR function imaing,and to provide more evidence for the evaluation and prediction of neoadjuvant chemotherapy. Methods 3.0TMR scan of mammary gland was performed before and after NAC on 22 breast cancer patients confirmed by biopsy pathology and accepted the NAC . Quantitative analysis was appllied on dynamic contrast enhanced MRI (maximal tumor diameter,time signal curve TIC and semi-quantitative parameters)and diffusion weighted imaging(ADC) and other index. All patients underwent surgery after the NAC. A correlated analysis on MRI evaluation of tumor residual lesion size and MRI parameter changes and pathological findings of maximum tumor diameter according to the pathology results was applied before and after the NCA. Results Effective group(n=18)in 27 patients,ineffective group(n=9). ADC changed in effective group was statistically significant(P<0.05),while in invalid group was not statistically significant(P>0.05);After the NCA, time - signal strengthen curve was gradually even by outflow-type platform type or ascending in effective group, differences are statistically significant in effective group(P<0.05),while in the ineffective group is not significant(P>0.05);Changes in the two sets of NAC dynamic enhanced scan before and after chemotherapy showed that semi-quantitative parameters in effective group was statistically significant(P<0.05),and ineffective group was not statistically significant(P>0.05);Between the two groups EER Emax Emax differences were statistically significant(P<0.05). Conclusions MRI function imaging can be used in monitoring changes in tumor volume and biological activity, in evaluating and predicting response to NCA,so it can be used for the preoperative evaluation of residual lesions after chemotherapy.%  目的探讨乳腺癌术前新辅助化疗(NAC)前后3.0TMR功能成像各项指标变化特点,为NAC的疗效评价与预测提供更多的依据。方法对27例经穿刺活检病理检查证实乳腺癌且接受NAC的患者行NAC前、后乳腺3.0TMRI扫描。对动态增强MRI和扩散加权成像(ADC值)等多指标定量分析。化疗结束后所有患者行手术治疗,取得病检结果。结果病灶ADC值有效组有统计学意义(P<0.05),无效组无统计学意义(P>0.05);有效组NAC后TIC渐趋平缓,可由流出型到平台型或上升型,其差异有统计学意义(P<0.05),而无效组没有统计学意义(P>0.05);两组NAC化疗前后动态增强扫描的半定量参数(EER、Tmax、Emax)的变化有效组均有统计学意义(P<0.05),无效组均无统计学意义(P>0.05)。两组组间比较EER、Tmax、Emax差异均有统计学意义(P<0.05)。结论 DWI结合动态增强TIC与半定量参数可早期预测与评估新辅助化疗疗效,为乳腺癌诊治提供一种安全、无创的检查手段。
