TTF-1和CK在肺癌中的表达及与Ki-67和p53的关系

         

摘要

目的探讨甲状腺转录因子-1(thyroid transcription factor-1,TTF-1)和广谱角蛋白(pan-cytokeratin, CK)在肺癌中的表达及TTF-1与Ki-67、p53的关系,为肺癌提供可能的分子诊断标志物。方法经病理证实的肺癌177例标本,免疫组化Envision二步法检测TTF-1、CK在其中的表达,同时检测Ki-67、p53,作相关性分析。结果 TTF-1在肺腺癌及小细胞癌中阳性表达率分别为87.0%和79.2%,CK在肺小细胞肺癌中阳性表达率79.2%。TTF-1、CK与患者年龄、吸烟、分化程度、淋巴结转移、TNM分期无明显相关性,TTF-1与患者性别相关;TTF-1与Ki-67呈正相关(P=0.008),与p53无明显相关性(P=0.225)。结论 TTF-1、CK有望作为肺癌临床检测标志物,可提高肺癌不同类型的检测诊断;TTF-1对肺癌预后可能有提示作用,尚需进一步探讨。%Objective The aim is to study the expressions of thyroid transcription factor-1(TTF-1)and pan-cytokeratin(CK)in lung cancer,and to investigate their relationship with Ki-67 and p53,and to explore the possibility as molecular diagnosis markers of lung cancer. Methods 177 cases of lung cancer confirmed by pathology from January 2008 to June 2012 in Zhejiang Hospital were reviewed. Envision two-step method of immunohistochemistry was used to detect the expression of TTF-1 and CK,and also Ki-67 and p53. The relationship between these markers and the clinico-pathological data were analyzed. Results TTF-1 was highly expressed in lung adenocarcinoma and small cell lung carcinoma(SCLC),and the positive rate was 87.0% and 79.2%,respectively. The positive expression rate of CK was 79.2% in SCLC. The expression of TTF-1 and CK were not correlated with patient age,smoking,differentiation,lymph node metastasis,TNM stage;TTF-1 was correlated with gender;TTF-1 was significantly correlated with Ki-67(P=0.008),without correlation with p53 (P=0.225). Conclusion TTF-1 and CK could be lung cancer prognosis markers for the detection of different types. Further studies are needed to evaluate the prognostic value of CK in SCLC.
