高血压病左心室几何构型改变对左心房壁心肌运动的影响

         

摘要

Objective To explore the influnces of myocardial motions of left atrial walls when the geometric configurations ofleft ventricular with essential hypertension (EH) were changed.Methods Methods294 cases patients with EH were selected.In thepatients, there were 87 cases in the left ventricular normal geometry (group II ) , 69 cases in the left ventricular concentric remodeling (group III), 57 cases in the left ventricular concentric hypertrophy (grouplV)and81 cases in the left ventricular eccentric hypertrophy (group V). In the same period , there were 45 cases in normal group(group I ) selected from crowds of physical examination. The average peak systolic velocity(APVs), average early diastolic velocity(AVe), average atrial systolic velocity(AVa), average peak systolic displacement(APDs), average peak systolic strain rate(APSRs),average peak systolic strain(APSs), average early diastolic strain rate (AEsr) and average atrial systolic strain rate (AAsr) were measured in every geometric configuration group of left ventricular with essential hypertention.results ResultsThe means of APVs were the group Ⅲ > the group IIand IV > the group I > the group V ;The means of APDs were the group II and Ⅲ > the group I and IV > the group V ;The means of APSRs and AVa were the group II , III and IV > the group I > the group V ;The means of APSs were the group I , II , IV > the group Ⅲ the group V ; The means of AVe were the group I > the group II > the group Ⅲ and IV > the group V ;The means of AESR were the group I > the group II , Ⅲ, IV and V , but the group II > the group Ⅲ; The means of AASR were the group II and IV > the group Ⅲ > the group I and V (P <0.05, respectively).concluslon ConclusionsThere were different influnces ofmyocardial motions of left atrial walls in different time phase of cardic cycle when the geometric configurations of left ventricular with essential hypertension were changed, their different motorial properties and functions of left atrial wall myocardium were showed.%目的 探讨高血压病左心室几何构型改变对左心房壁心肌运动的影响.方法 选取高血压患者294例,其中正常构型者87例(组Ⅱ),向心性重构者69例(组Ⅲ),向心性肥厚者57例(组Ⅴ),离心性肥厚者81例(组Ⅴ).另择同期正常体检人群45例(组Ⅰ).测定比较各组左心房壁平均收缩期峰值速度(APVs)、平均舒张早期速度(AVe)、平均舒张晚期速度(AVa)、平均收缩期峰值位移(APDs)、平均收缩期峰值应变(APSs)、平均收缩期峰值应变率(APSRs)、平均舒张早期应变率(AEsr)及平均舒张晚期应变率(AAsr).结果 APVs:组Ⅲ>组Ⅱ、Ⅳ>组Ⅰ>组Ⅴ;APDs:组Ⅱ、Ⅲ>组Ⅰ、Ⅳ>组Ⅴ;APSRs、Ava:组Ⅱ、Ⅲ、Ⅳ>组Ⅰ>组Ⅴ;APSs:组Ⅰ、Ⅱ、Ⅳ>组Ⅲ>组Ⅴ;AVe:组Ⅰ>组Ⅱ>组Ⅲ、Ⅳ>组Ⅴ;AESR:组Ⅰ>组Ⅱ、Ⅲ、Ⅳ、Ⅴ,组Ⅱ>组Ⅲ;AASR:组Ⅱ、Ⅳ>组Ⅲ>组Ⅰ、Ⅴ(均P<0.05).结论 高血压病左心室几何构型改变对左心房壁心肌运动在心动周期的不同时相产生了不同的影响,使其显示不同的运动特性及心肌功能.
