The NTSB says the FAA and Gulf-stream missed opportunities in 1987 to flag inadequacies with a safety system meant to prevent the Gulfstream IV throttles from moving when the gust lock is engaged. Nearly three decades after the business jet's certification, the flaw manifested in a fatal crash on May 31, 2014, when pilot errors necessitated a safety net that did not perform as intended.
