The blatant underuse of checklists and communications best practices by the crew of a Gulfstream GIV that crashed after a flawed takeoff attempt at Hanscom Field in May 2014 has the NTSB asking whether this is a broader problem in the industry. The safety agency determined that the accident, which killed all seven on board, was primarily caused by the pilots' failure to disengage the gust-lock system, a checklist item, or to subsequently catch the error through a flight-control check, also on the checklist (see related article above). The cockpit voice recorder revealed that the challenge-verification-response (CVR) method, a best practice generally used in two-pilot cockpits, was absent from the Hanscom crash crew's standard operating procedures that night. With CVR, one pilot will call out an item to be checked and the other pilot will take an action and call out a response.
