There is increasing concern worldwide about the occurrence of Enterobacter sakazakii in milk-based powdered infant formula. This organism has been responsible for cases of meningitis, septicaemia, necrotising enterocolitis and death in premature or other immuno-compro- mised infants, and a number of outbreaks within this patient group have been reported over the last few years in neonatal intensive care units around the world. The recognition that these outbreaks were linked to milk-based powdered infant formulas resulted in the release of a United States Food and Drug Administration (USFDA) alert to health care professionals, which was revised in October 2002. Risks associated with milk-based powdered formulas, human milk fortifiers (for pre-term breast milk) and soy-based powdered formulas are due to the non-sterile nature of these products. They are not processed at high enough temperatures, or for sufficient time, to achieve commercial sterility and therefore there is a risk they may remain contaminated and pose a threat to susceptible infants.
