Ourgoalwastodevelopamodelforthestudyofmaternaladrenalglandregulationandtheeffectsofmaternalcortisolsecretiononfetalhomeostasis.Atabout108daysofgestation,beforethetimeofrapidfetalgrowthorfetaladrenocorticalmaturation,ewes,underhalothaneanesthesiawithcontrolledventilationandpositionedinsternalrecumbency,wereadrenalectomized.Ewesweretreatedwithaldosteronebyintravenousinfusion(3g/kgofbodyweightperday)toinducenormallate-gestationaldosteroneconcentration.Eweswerealsotreatedwithcortisol;for2postoperativedays,thisinfusion(1to2g/kgpermin)inducedplasmaconcentrationsimilartothatassociatedwithstress.Thereafter,thedosageofcortisolwasreducedtoinduceplasmavaluessimilartonormallate-gestationcortisolconcentrationinewes(1mg/kgperday),ortovaluesinnonpregnantewes(0.6mg/kgperday).Administrationofcortisolandaldosteronewasrequiredtopreventelectrolyteimbalanceandsignsofhypoadrenocorticism.Withsteroidreplacement,plasmaprotein,electrolyte,andglucoseconcentrationsinadrenalectomizedeweswerenotdifferentfromthoseinsham-operatedpregnantewes.Of11adrenalectomizedewes,onediedasaresultoffailureoftheinfusionpump,andonediedasaresultofinappropriatetreatmentforhypoglycemia.Oftheremainingewes,twoabortedfetuses,threeeweseachdeliveredoneliveandonedeadfetus,twodeliveredlivesingletonfetuses,andtwodeliveredtwins.Therefore,thismodelofrelativehypoadrenocorticisminpregnancyisfeasibleandpracticalforstudyingtheinfluenceofmaternalcortisolconcentrationonmaternalandfetalhomeostasis.
