Intensiveinsulintherapycanleadtohypoglycemia,withpatientssometimesdevelopinghypoglycemicneuropathy.SpontaneouslydiabeticWistarBonnKobori(WBN/Kob)ratsdevelopdiabeticperipheralmotorneuropathycharacterizedbysegmentaldemyelinationandaxonaldegeneration.Weexaminedtheshort-termeffectsofhypoglycemiaonneuropathicchangesintheserats.SpontaneousdiabeticWBN/Kobratsreceivedinsulinimplantsfor40dandweredividedinto3groupsbasedonbloodglucoselevels:groupN,normoglycemictoslightlyhyperglycemic(150to250mg/dL);groupH,hypoglycemictoslightlyhyperglycemic(50to200mg/dL);andgroupD,nontreatedspontaneouslydiabetic(350to420mg/dL).Conductionvelocitywasmeasuredinsciatic#8211;tibialmotornerves;thesenervesalsounderwentqualitativeandquantitativehistomorphologicanalysis.ConductionvelocitywasnotsignificantlydifferentinN,D,andHgroups.MorphologicanalysisofthesciaticnervesofHratsshowedseverechanges,includingaxonaldegeneration,myelindistention,andendoneurialfibrosis,thattendedtooccurinlarge,myelinatedfibers.NandDratsshowedrelativelymildchanges.ThedegreeanddistributionofdegeneratednervefibersinHratsweresignificantlyhigherthaninNandDrats.Theseresultssuggestthathypoglycemiaoflessthan50mg/dLinducedsevereperipheralneuropathy.HypoglycemiclesionsdifferedfromthehyperglycemiclesionsindiabeticWBN/Kobrats.Thisratstrainisanappropriatemodelforinvestigatingthehypoglycemicperipheralneuropathythatcanbeassociatedwithadiabeticcondition.
