ThenaturalhistoryforinhalationalBacillusanthracis(Amesstrain)exposureinNewZealandwhiterabbitswasinvestigatedtobetteridentifypotential,earlybiomarkersofanthrax.TwelveSPFBordetella-freerabbitswereexposedto150LDsubxmlns="http://pub2web.metastore.ingenta.coms/"50/subaerosolizedB.anthracisspores,andclinicalsigns,bodytemperature,completebloodcount,bacteremia,andpresenceofprotectiveantigenintheblood(thatis,antigenemia)wereexamined.Thedevelopmentofantigenemiaandbacteremiacoincidedandprecededbothpyrexiaandinversionoftheheterophil:lymphocyteratio,anindicatorofinfection.Antigenemiawasdeterminedwithin1hbyelectrochemiluminescenceimmunoassay,comparedwiththe24-htraditionalcultureneededforbacteremiadetermination.Rabbitsappearedclinicallynormaluntilshortlybeforesuccumbingtoanthraxapproximately47hafterchallengeorapproximately22hafterantigenemia,whichsuggestsarelativelynarrowtherapeuticwindowofopportunity.Toevaluatethetherapeuticrabbitmodel,B.anthracis-exposedrabbitsweretreated(afterdeterminationofantigenemiaandlaterconfirmedtobebacteremic)intravenouslywiththefluoroquinoloneantibioticlevofloxacinfor5datatotaldailydoseof25or12.5mg/kg,resultinginnearly90%and70%survival,respectively,tothestudyend(28dafterchallenge).Thepeaklevelfor12.5mg/kgwasequivalenttothatobservedfora500-mgdailylevofloxacindoseinhumans.Theseresultssuggestthatintravenouslevofloxacinisaneffectivetherapeuticagainstinhalationalanthrax.Takentogether,ourfindingsindicatethatantigenemiaisaviableandearlybiomarkerforB.anthracisinfectionthatcanbeusedasatreatmenttriggertoallowfortimelyinterventionagainstthishighlypathogenicdisease.
