To the Editor: Pertussis, orwhooping cough, is a highly conta-gious respiratory disease in humanscaused by Bordetella pertussis.Reemergence of pertussis has beenobserved in many countries with highvaccination coverage. In the UnitedStates, reported cases of pertussis inadolescents and adults have increasedsince the 1980s, despite increasinglyhigh rates of vaccination in infantsand children (1). At the same time,clinical B. pertussis isolates havebecome antigenically divergent fromvaccine strains (2,3). This observationhas raised the question of whethervaccination has caused selection forthe variant strains, and whether thereemergence of pertussis in vaccinat-ed populations is due to vaccinationnot protecting against these antigenicvariants as effectively as it protectsagainst vaccine type strains. On theother hand, vaccine-induced immuni-ty wanes over time, and pertussis isnot only a childhood disease but alsoa frequent cause of prolonged illnessin adults and adolescents today (4).
