A 61-year-old man presented with a 1-week history of right upper quadrant pain. He had been otherwise healthy until 6 monthspriortoadmission,whenhehadaweightlossof20kg. One month prior to admission, he started to exhibit poor appetite and general malaise but no nausea, vomiting, or diarrhea. Three days before admission, abdominal pain, disturbed consciousness, and incontinence were noted. The patient was brought to the emergency room, and laboratory examination revealed the following findings: g-glutamyl transpeptidase 122 U/L; alkaline phosphatase 173 U/L; and serum potassium 2.8 mmoI/L. Abdominal sonography revealed a distended gallbladder with sludge and air and dilation of the bilateral intrahepatic ducts. Endoscopic retrogradecholangiopancreatography revealed commonbile duct stones.
