More than 90% of collisions, over 80% of tanker accidents, 79% of towing vessel groundings and 75% of fires and explosions are attributed to human error. Such statistics would suggest that project managers and designers within the maritime industry are still unaware or ill prepared to consider human factors engineering (HFE) as a critical part of the design process.
