Various studies have shown that on average the surgical treatment of a freshly ruptured Achilles tendon can be considered as more appropriate than a conservative immobilization therapy. On the other hand, the rate of complications in open reconstructions is quite considerable at 11-29%. Therefore it seems reasonable to develop a method which reduces the risk of complications arising from surgery and simultaneously allows early postoperative treatment encouraging maximum mobilization. We developed a percutaneous ten-odesis of the Achilles tendon, using two Len-gemann extension wires and fibrin sealing of the point of rupture since 1995, this method has been applied in twenty-one patients; the postoperative observation period was one year. All patients were male, their average age was forty-two years. The Achilles tendon ruptures occurred during sports activities and were treated by surgery within twenty-two hours on average. In one case re-rupture due to a trauma occurred, all other results were good to very good and free of complications. Percutaneous tenodesis with Lengemann extension wires and fibrin sealing is a simple, cost-effective and reliable method which should be applied in the treatment of fresh Achilles tendon rupture. Especially the easy handling, good results and high acceptance among the patients recommend the method described here for routine clinical application.
