The pilot reported to work at 2:00 a.m. for a Part 135 car-go flight in a Piper Lance. The flight from Burbank, Calif., to Phoenix, Ariz., took four hours, after which he shuffled off to the company apartment for crew rest. The NTSB figures he got about five hours of sleep before waking at noon. His flight back to Burbank wouldn't leave until almost 11 that night, but he didn't go back to sleep. Maybe he figured five hours was enough. Maybe he planned to nap later. Maybe he was too tired to realize he was tired. At 22:30 PST, 35 minutes into the return flight, he flew a perfectly good airplane into the side of a mountain 10 nm south of course. No deficiencies were found in what was left of the aircraft. Official probable cause was, "Incapacitation of the pilot, due to inadequate sleep."
