In response to the article, "The Lesser of Two Adverse Reactions," the Institute for Safe Medication Practices (ISMP) would like to comment on Chakraborti and Egan's evaluation of the ISMP's recommendations regarding the safe use of promethazine. The authors reported a severe cardiac adverse event that occurred in a patient after receiving the anti-emetic ondansetron intravenously (IV). The hospital had banned the use of IV promethazine, which had previously been widely used as an anti-emetic, after ISMP described cases of tissue necrosis and local injuries from IV promethazine.
