The need to improve quality of care and patient safety has become the new imperative, driven by current health care reform initiatives as well as the ethical requirement to do what is best for our patients. While there are no quick fixes for this problem, there are empirically tested methods that work. We know because we have implemented them.rnFirst, health care leaders must create a clear "chain of accountability" among senior leaders that holds frontline clinicians accountable for patient outcomes.
