Priority setting in health research is an important component of national health research strategy development. With limited resource, priority setting helps policy makers in identifying areas of research that they will invest to ensure the best value for money. It also helps co - ordinate researchers to focus their actions into important and meaningful areas. This article describes the health research priority setting study that was conducted in Thailand in 2006 by a collaborative network of scholars from all regions of the country. The process includes extensive literature review, data collection, and questionnaire surveys to health practitioners and health researchers in Bangkok and other regions, both in the academic sector and in the Ministry of Public Health. The aim of the study was to identify the top 10 diseases that should be the priority for health research investment in Thailand. In addition, the study also explored the types of research gap in each disease whether they are from the lack of knowledge, the lack of tools and technology, or the lack of effective service delivery system. Specific research topics for each of these 10 diseases were also identified. The study reveals that the priority diseases ranked according to their importance are HIV / AIDS, traffic accident, diabetes mellitus, stroke, cardiovascular disease, chronic obstructive pulmonary disease, liver cancer, alcohol dependency, depression, and tuberculosis. There is a strong correlation between the disease rankings for priority research investment and the disease rankings from the Ministry of Public Health's burden of disease study. In the subgroup analyses, it was found that the priority research rankings are not the same for respondents from different occupation groups or different regions. The study also identifies several cross - cutting issues that warrant research investment including health education and behavioral change techniques, health information system development on disease epidemiology and health risks, health resource distribution study especially the distribution of high - cost technology and the health workforce, and research on development of proactive health service system. This study shows that it is important for all stakeholders to be given an opportunity to participate in the priority setting process of health research. The findings from this priority setting process should be utilized by research agencies on budgeting decisions or research granting decisions. The authors recommend active coordinations among all research funders and stakeholders to support high priority diseases together. It is also suggested that further investment to develop the tools and process for health research priority settings is necessary for progressive and sustainable development of national health research strategy.
