Decreased Distribution of Airway Lymphatics in Individuals with Fatal Asthma

I previously reported the phenotypic distribution patterns of airway smooth muscles in individuals with fatal asthma (individuals with Type I asthma with smooth muscle bundle thickening only in large airways and Type II in whole airways), in comparison with normal control lungs, by different mechanisms of airway smooth muscles, hypertrophy, and/or hyperplasia (Am Rev Respir Dis 1990;141:1322,1327, and 1993; 148:720). We hypothesized that increased smooth muscle bundles in the airway walls would disrupt airway lymphatics to impair airway clearance in these individuals with fatal asthma. Methods: The autopsy lungs of seven individuals with fatal asthma (three with Type I, four with Type II asthma) and five control subjects were examined by immunohistochemistry to reveal the lymphatics distributed in the airway walls. The total area of lymphatics around each cross-sectioned airway was measured and its airway radius was calculated using an image analyzer system. Results: The total area of airway lymphatics in each lung was found to be positively correlated with the airway radius (R). The distribution areas of lymphatics in larger airways of individuals with both types of asthma were significantly decreased compared with those of control subjects, and individuals with Type I asthma contained much less lymphatics in these airways than did individuals with Type II asthma.
