I was reading some publications by ASSE professional members and one caught my eye. Murder by Electrocution, by David MacCollum, is outstanding, especially when considering the author's background in the safety profession. He is an ASSE Fellow and served as 1975-76 ASSE President. In addition, MacCollum has 60 years of engineering practice that involves the realities of design engineering. His book enlightens readers with this important story of bridging the gap between constant change and innovation in construction and the need to communicate engineering principles, all in a language the public can understand. Throughout his career, much of MacCollum's work has been dedicated to dispelling the entrenched belief that failure (an incident) can be either partly or completely attributed to the worker's action, inaction or mistakes. This predisposition is commonlv referred to as human error.
