In the hours immediately following any major safety incident, there is a typical rush to judgment. Media reports broadcast the inevitable finding-many reported incidents are, at least initially, attributed to human error. Hasty judgments promote the growth of a culture of blame. Placing blame is a superficial-and at best temporary-fix to most safety sustainability problems.rnThe practice of HPI encompasses an inquiry process that is deep and broad, circular rather than linear, ensuring that true root causes of human error are exposed and solutions are appropriate and permanent.
