MS completed his radical radiotherapy, despite his alcoholism, defaulted appointments, non-compliance with radiographers' instructions to shave, and confusion with medication. He had poor oral hygiene and dentition, therefore weight loss of over a stone was not surprising. He experienced many of the early and late radiotherapy reactions seen with patients, but the support he received during his radiotherapy helped him to complete his course. Skin reactions can be particularly severe when treating patients with head and neck tumours, as the exit dose of radiation is high and the uneven contour throughout the treatment area results in a varied skin dose. The immobilisation mask and his beard may have compromised the skin sparing effect of megavoltage treatment. It may be beneficial for the department to invest in an electric razor or trimmer. Clothing, and his refusal of dressings, irritated his moist desquamation. Therefore, body-shaped dressings may have been easier for him to use, and it is felt that the department should invest in some for future head and neck patients. As was expected his skin recovered by his first follow up, a month after completing treatment, the normal recovery period for acute reactions. The support from which MS benefited consisted of repeated advice given verbally, and in writing, mouth care packs and saline sachets, dressings, analgesics, and topical treatments, nutritional support, and a multi-disciplinary approach. He had been seen regularly by his clinician, radiographers, a dietician, head and neck specialist nurses, and reception staff. In addition to this he was offered transport to ensure that he kept to his treatment plan.
