Fan et al. (2012) describe the results of annual influenza vaccination on reducing mortality and hospitalization in elderly patients. I agree that the main topic is very interesting and important research which will have a potential impact on public health. But for meaningful discussion to take place, I believe that the results should be expressed more clearly. The authors are correct in pointing out total deaths in the influenza/ non-influenza seasons and calculating hazard ratios with confidence intervals. But in such a cohort study we must state the numbers of lost to follow up (Campbell et al., 2007a). In their analyses, they used Cox proportional hazards models, but they did not mention censored data (Campbell et al., 2007b). In Table 1, which presents the baseline characteristics of patients, some variables (like gender and hypertension) are significantly different between the vaccinated and non-vaccinated groups. But in subsequent analyses, these variables are not considered in the modeling process. If they were, the results might change.
