A 19-year-old man presented with auditory hallucinations and paranoia. His psychotic symptoms were of 8 weeks' duration and were preceded by more than 1 year of social isolation and poor functioning. He was hospitalized and diagnosed with schizophrenia. Organic causes were ruled out, and risperidone was started. He was treated with risperidone 6 mg daily and was referred to psychotherapy. Although his positive symptoms were well-controlled, he remained isolated with limited social interactions. During psychotherapy sessions, he admitted to fear of going outside because of anticipatory anxiety of having a relapse of his psychosis, losing control in the street, and being ridiculed. He also reported panic attacks. He did not report any depressive symptoms, and he said he was not abusing any drugs.
