Worldwide, diabetes is an important public health problem, and its impact on healthcare financing is of considerable concern. It is apparent from studies in several Western countries that individuals with diabetes have increased medical expenditure compared with individuals without diabetes, as a consequence of the disorder itself and also because of the severe macrovascular and microvascular complications associated with diabetes.Japan has a large number of patients with diabetes, and the number of Japanese patients with end-stage renal disease due to diabetic nephropathyis increasing. End-stage renal disease is the most costly diabetes-related disease; patients with this condition require long-term dialysis which is an expensive medical service. In addition, diabetic retinopathy is a major cause ofacquired blindness in Japan. Information on medical expenditure for diabetic patients is scarce in Japan and other Asian countries. Therefore, in order to determine the expenditure of diabetic patients, a 10-year follow-up study was performed in a population in Japan, where many medical services are provided by the public medical insurance system.
