TO THE EDITOR: The article by Kornfeld et al., is an important contribution to the literature of discharges against medical advice (AMA), an area desperately in need of more quality empiric data. The authors report that they classified a C-L consultation as successful, for example, if the patient ultimately remained in the hospital after initially planning on leaving AMA. Given the authors' conclusion that ero-sion of the physician-patient relationship may be responsible for an increase in AMA discharges, the decision to characterize successful consultations as such was noteworthy. Likewise, the authors use of the phase "threatening to sign out AMA" suggests that patients' preferences around discharge decisions are uniformly aggressive and inappropriate, a conclusion not supported by the presented data. If patients and physicians are to achieve the ideal of a shared decision-making process, a successful outcome surrounding an AMA decision should be measured not simply by whether the patient acquiesced to the physician's recommendation, but by whether a competent patient's preferences and values were elicited and followed.
