We report the case of a man admitted to a hospital for community-acquired pneumonia, who developed closed-eye hallucinations, and discuss the relationship of lidocaine (used as a local anesthetic) and the onset of visual hallucinations. Mr. S, a 43-year-old man with no psychiatric history, presented with complaints of shortness of breath, productive cough, headache, muscle aches, and fever. He denied any previous medical problems or current medications. His breathing was labored, with a respiratory rate of 30 breaths/minute. His laboratory values were significant for leukocytosis (WBC = 17.5 k/mcl); a urine drug screen was negative. A presumptive diagnosis of community-acquired pneumonia was made, and he received intravenous (IV) ceftriaxone and oral azithromycin in the emergency room and was admitted for continued treatment.
