摘要

Cervical Spondylotic myelopathy is a common progressive disorder of the aging cervical spine. Surgical decision-making for the treatment of this disorder must be based on a fundamental knowledge of primary degenerative and secondary com-pressive and ischemic processes. A patient's clinical status, spinal geometry and bio-mechanics, and imaging findings help provide a logical surgical plan, Dorsal approaches, including decompressive and detethering procedures, with or without subsequent fusion, are described in this article. The dorsal approach to the cervical Spondylotic spine is simple, straightforward, and fraught with few surgical complications when appropriately applied. In many patients, a dorsal approach meets the ana-tomicopathologic criteria for the decompressive treatment of cervical Spondylotic myelopathy.
