Ossification of the posterior longitudinal ligament (OPLL) is the disease in which patients gradually (or suddenly, in some patients) develop neurologic deficits of four extremities. These deficits can be partially reversed or even cured by surgical treatment. Described in this text are the important technical points for operating on OPLL.
