The molecular era is upon us! The complexity of testing and management of information has led to targeted therapeutics and personalized medicine. There is greater emphasis on sophisticated diagnostic modalities and multidisciplinary approach to patient management. Cytopathology must adapt and is adapting to meet the needs of this rapidly changing environment and the increasingly dynamic relationships with our clinical and radiology colleagues. The basic cytologic criteria for primary pulmonary carcinomas have remained relatively static over the last decade. Now there is increasing pressure to locate and diagnose pulmonary lesions early and to collect material for Bronchoscopically and radiologically guided fine needle aspirations have been the mainstays of procurement for pulmonary specimens; however, their ability to visualize and sample lesions in the lung has advanced dramatically and has highlighted the need for greater interaction with radiologists and pulmonologists. This was the subject of the State of the Art Symposium at the 56th Annual Meeting of the American Society of Cytopathology in November 2008 in Orlando, Florida. This issue of Pathology Case Reviews continues this dialogue and highlights current concepts in the detection, diagnosis, and management of lung cancer.
