Role of wire-guided cannulation in the endoscopic retrograde cholangiopancreatography.

We read with great interest the meta-analysis by Cennamo et al. (1).In this study, five randomized controlled trials were included to compare the primary cannulation rates between two groups, i.e., wire-guided cannulation vs. standard cannulation. Among those trials, four used the sphincterotome-based technique and one used a standard catheter without sphincterotome in the contrast group, respectively. In our routine endoscopic retrograde cholangio-pan-creatography (ERCP) practice, using a sphincterotome without a guidewire has a higher primary success rate for cannulation than using a standard catheter, which is in line with a previous study (2).
