This Christmas we once again made our annual visit interstate to visit my father-in-law. He is elderly, lives alone and has hypertension, atrial fibrillation (AF) and chronic renal failure.On arrival we found he had chronic diarrhoea, was dehydrated and had to be admitted to hospital with acute renal failure.After a diabolical experience in hospital where medications were either incorrectly given or not given at all, and no records were kept, we brought Jack home. Thank goodness as a pharmacist I could sort out the medication mess and act as Jack's advocate while he was in hospital. How the rest of the patients managed, I hate to think.
