Axillary lymph node involvement continues to be the most powerful predictor of breast cancer outcome. Over the last decade, the sentinel node biopsy technique has replaced routine axillary lymph node dissection for staging the axilla. The use of both vital blue dyes and radiocolloid results in the highest rate of sentinel node identification. Herein we describe a potential pitfall in the identification of the true sentinel node, secondary to migration of tattoo dye into an axillary lymph node.A 57-year-old female underwent partial mastectomy and sentinel lymph node biopsy of the left axilla performed after injection of technetium-99m unfiltered sulfur colloid and intraoperative subareolar injection of methylene blue. She had a large multicolored tattoo of the left arm and shoulder. At surgery, an axillary "hot-spot" was identified upon transdermal scanning of the axilla. A blue, non-radioactive lymph node was detected in the superficial axilla. Deeper in the axilla, a tan-colored radioactive node was identified. Additional palpable nonsentinel nodes were also removed. Frozen section evaluation of the two "sentinel" nodes showed no evidence of metastasis but was remarkable for tattoo pigment deposition in the blue node (Figs. 1-3). Subsequent IHC staining revealed microm-etastatic disease in the radioactive node; the blue node and additional seven lymph nodes were negative for metastatic disease.
