Cardiovascular disease (CVD) is the second leading cause of mortality in Japan, and its prevention is an urgent public health issue. Conventional risk factors for CVD include hypertension, diabetes mellitus, dysli-pidemia, and obesity, and smoking is also considered a modifiable risk.Ridker and colleagues presented the Reynolds Risk Score (RRS), designed to predict the cardiovascular risk over the following 10 years in patients without diabetes. As compared with the Framingham Risk Score (FRS), some biochemical variables are also included in the calculation of the RRS, although individuals with diabetes mellitus are not suitable for the application of either of the risk scores.Comparative studies5'6 have revealed the advantages and limitations of risk scores. The development of a risk score for predicting CVD is useful for recommending risk-lowering actions for each individual, such as smoking cessation, regular exercise habit, dietary regulation of calorie, and salt intake. Because the incidence of CVD is related to genetic factors, RRS includes a family history of CVD. It is also speculated that RRS may be related to the metabolic syndrome, because some components are overlapping.
