We report a possible interaction of potential clinical significance between milnacipran and warfarin. Milnacipran is a serotonin-norepineph-rine reuptake inhibitor (SNRI) that is eliminated by direct renal excretion; it is thought to interact with the cytochrome P450 system to a minimal extent.A 53-year-old man receiving warfarin for an aortic valve replacement was admitted to Tokyo Women's Medical University, Japan, for depression; his pro-thrombin time-international normalized ratio (PT-INR) had been stabilized with warfarin 2 mg/d for many years. Milnacipran was started and increased in a stepwise fashion (Table I).
