Lamotrigine (LTG) is an antiepileptic drug that was introduced for adjunct treatment of partial seizures in Western countries in the 1990s and in Japan in 2008. Here, we report three cases of drug eruption (DE) due to LTG that showed a positive lymphocyte transformation test (LTT). In case 1, a 62-year-old Japanese woman with a history of glioblastoma was started on LTG 50 mg/day to prevent epileptic fits. Twenty days later, she noticed multiple erythemas over the trunk and extremities (Fig. 1a). Laboratory tests showed elevation of eosinophil percentage (10.0%) and C-reactive protein (CRP) (0.43 mg/dL). The clinical and histopathological findings (Fig. 1a,d) were compatible with maculopapuiar eruption (MPE). LTG was stopped. The eruptions disappeared in 11 days with p.o. administration of epinastine hydrochloride 20 mg/day and topical application of 0.05% difluprednate. A positive LTT (stimulation index [SI], 2.9; normal, <1.8) with LTG was obtained using blood samples taken 11 days after the eruption onset.
