Psoriasis vulgaris is recently associated with other comorbidi-ties like metabolic syndrome and cardiovascular disease. Some studies have also shown connection to celiac disease, a multisystemic disorder associated with skin manifestations like dermatitis herpetiformis Duhring as well as the systemic symptoms, A 48-year-old man with a history of psoriasis vulgaris had been followed at our department since 2006 when he was referred to us for initiation of a biologic therapy. He suffered from arterial hypertension, hyperlipidemia, hyperuricemia andnon-alcoholic fatty liver disease (NAFLD). He used to smoke 10 cigarettes daily. Before biologic therapy, he was treated with methotrexate and with ultraviolet B (311 nm). Five years prior, therapy with efalizumab was initiated for 12 months but was then switched to etanercept for 2.5 years the therapy and finally to adalimumab for 1.5 years due to loss of response. After relapsing on adalimumab, we decided to change from anti-tumor necrosis factor-alpha treatment to monoclonal antibody against interleukin-12/-23. Therapy with ustekinumab was initiated at a dose of 90 mg, because of his weight (108 kg), with excellent results.
