There is evidence that greater hemo-dynamic response (eg, increase in heart rate and blood pressure) during ECT is associated with greater therapeutic benefits. This is linked to possible generalization of seizure in central autonomic network.5 It would be of interest to see if there is any difference in the hemodynamic response between BFECT and BTECT. In a study comparing the efficacy of BFECT and BTECT in schizophrenia (V H. Puthane et al, unpublished data, 2012), we examined this issue. Of 122 patients who were randomized to receive either BFECT or BTECT, 114 completed the study. During the first ECT session, titration method was used to calculate seizure threshold. Subsequent stimuli were delivered at 1.5 times the threshold level. All ECTs were administered under anesthetic modification (thio-pentone, 2-4 mg/kg, and succinylcholine, 0.5-1 mg/kg)
