Although strong reservations have been expressed about unmodified electroconvulsive therapy (ECT), its practice seems widespread. Recognizing, perhaps, that the problem will not go away in a hurry, in this issue of the Journal, Gallegos et al review the efficacy and drawbacks of unmodified ECT and offer ideas for the minimization of adverse outcomes associated therewith. Some of the points in their article merit examination.
