There are good arguments on both sides, as there usually is in a controversy:1 It is true that as a student's Health Education Systems, Inc. (HESI) Exit Examination score increases, the probability of that student passing the NCLEX-RN? increases. This is a statistically significant association.2 It is true that the HESI Exit Examination's predictive ability is low.Resolving these two true, yet seemingly contradictory, statements is at the heart of the problem. If the arguments were stated in another, completely complementary fashion, these statements would read as follows:3 It is true that a student with a high HESI Exit Examination score (A/B) is likely to pass the NCLEX-RN (pass rate = 0.983).4 It is true that a student with a lower HESI Exit Examination score (C, D, E/F) is likely to pass the NCLEX-RN (pass rate = 0.855).5 It is true that a student with a lowest HESI Exit Examination score (G/H) is likely to fail the NCLEX-RN (fail rate = 0.502).The critical point here is that statement #4is difficult to deal with when the predicted outcome is dichotomous and the probabilities of the two outcomes are very unequal (passing rate = 85%). Spurlock and Hanks (2004) very rightly point out that students with C, D, or E/F HESI Exit Examination scores fail the NCLEX-RN at low rates. And Nibert et al. (page 303-307) very rightly point out that students with A/B scores pass the NCLEX-RN at high rates.The only way to resolve this issue is to look at the outcomes of using the HESI Exit Examination. It was stated that for those schools that implemented a progression policy, school pass rates for the NCLEX-RN increased.
