Quality of life (QOL) outcome measures continue to dominate efficacy trials in urogynecologic manuscripts. This prospective QOL questionnaire study makes a significant contribution to understanding what drives a patient to select reconstructive surgery for prolapsed, rather than conservative management with pessary or observation. Participants were women referred for subspecialist evaluation, all of whom completed the questionnaire before evaluation and counseling with the physician. Over a 5-year period, 680 women completed the questionnaire and 251 chose surgery. The Sheffield Prolapse Symptoms Questionnaire was used to review general, urinary, bowel, sexual, and QOL domains. About two-thirds of women chose nonsurgical management with pessary. Overall, 89% of women perceived a vaginal lump, 82% reported urinary urgency, and 53% fecal urgency, with 67% and 62% reporting difficulty emptying bladder or bowels, respectively, and 68% reporting dragging pelvic pain. Women choosing surgery were younger at 58 years mean age vs. 66 years for pessary patients, with greater degrees of dragging pain, needing to reinsert the prolapse manually and difficulty emptying bowels. The greatest difference by far, however, was the impact on body image and sexual functioning.
