Bone grafts are a useful tool in the armamentarium. of the head and neck reconstructive surgeon. They can be used in many aspects of reconstructive surgery ranging from primary palate clefts and alveolar augmentation to repair small bony, mandibulectomy defects. The sources for bone grafts are numerous, but the most commonly utilized in the head and neck region are calvarial split- or full-thickness bone, iliac crest, and irradiated autologous bone. One of the essential criteria in selecting the type of bone grafts is the donor site morbidity and ease of harvest. This criterion generally makes many surgeons choose irradiated autologous bone grafts over autogenous bone grafts. Split-calvarial bone grafts can be technically difficult whereas iliac crest grafts have a prolonged recovery phase. The disadvantages of the autologous bone grafts include expense and risk of infections, namely HIV and hepatitis C.
