In 2000, use of fresh frozen plasma (FFP) within the West of Scotland (WoS) was rising at 2% per annum. Published studies suggested that most indications for FFP use were thought to be inappropriate (Thomson et al., 1991; Jones et al., 1998). As a result, a regional audit to determine FFP and cryoprecipitate (cryo) use was carried out in 2000 under the direction of the West of Scotland Consultant Haematologists' Audit Group and results published in this journal (Soutar et al., 2004). This group encompasses the area supported by the WoS Regional Transfusion Centre, a population of approximately 3 million serviced by 15 hospitals. The main findings of this study were disproportionate use of FFP in tertiary centres (by a factor of almost 2) compared with district hospitals, and indications for use of FFP were out with British Committee for Standards in Haematology (BCSH) guidelines in 32% of issues. This initial study stimulated a local practice review in outlying hospitals to address their transfusion practices, and it was also suggested that each hospital develop its own protocol for FFP and cryo replacement. We set out to re-audit blood product use and practice within the WoS to assess the impact of the first audit. Unfortunately, because of the time-consuming nature of the second-stage examining indications for FFP use, this was not repeated during this audit cycle.
