Audits of warfarin use in both hospital and community settings in Auckland, New Zealand have led to initiatives to improve care for patients prescribed warfarin. Development of warfarin prescribing guidelines,1 improvement of information contained in the electronic discharge summary, production of an informative video for patients, and training sessions for healthcare practitioners are some of the initiatives undertaken.The initial audits aimed to assess the quality of warfarin prescribing in the inpatient setting, and to examine the quality of information transfer to general practitioners (GPs) and patients during the discharge process. A questionnaire was then sent to GPs and community pharmacists (CPs) to review the use of warfarin in primary care, with the ultimate aim to improve care of patients on warfarin across all practice settings.
