Prescription and administration errors leading to adverse drug events are often preventable,,1 However few studies have investigated prescribing errors in UK psychiatric hospitals.2'3 At Whitchurch Hospital, staff had identified irregularities on both the prescription and administration records used on the wards. Therefore permission was granted for an audit to review prescription and administration records against trust guidance for both prescription writing and administration recording.4 The aim of the audit was to identify the number and type of prescribing and administration irregularities on a sample of wards.
