In 1997, we conducted a study showing how over-the-counter (OTC) low-dose aspirin was being used by pharmacy clients.1 The majority of patients took aspirin as a consequence of cardiovascular (CV) disease with the knowledge of their general practitioner (GP); however about 15% self-treated with aspirin without informing their doctor. In many such cases there was no apparent history of CV disease, and over 1.3% of 602 patients had potential upper gastrointestinal (GI) side-effects.Augustovski investigated the outcomes of patients taking low-dose aspirin, stratified by risk-factor (e.g. diabetes, angina) and showed a positive risk-benefit for patients with CV risk factors, but excess mortality in those without recognised risk factors.2 Since the mid-to-late 1990s, low-dose aspirin is routinely prescribed in CV disorders to reduce the risk of major events. Aspirin has also been proposed to prevent travellers' thrombosis, although this is unlicensed 3 The aim of this study was to examine how OTC low-dose aspirin use by pharmacy clients has developed over the past 6-7 years.
