At this time of healthcare transformation, the role of nurses in community-based healthcare is emerging as a critical asset in achieving the Triple Aim. At the same time, population health competency has become a central variable in the successful transformation of healthcare. With the enactment of the Patient Protection and Affordable Care Act and the dissemination of the Institute of Medicine (IOM) Report on the Future of Nursing, nurses are increasingly being called on to exercise this competency in developing innovative practice models and health systems that drive patient-centered, community-centric care.
